Xbox Disconnect and crashes when upgrading gear

I have a few issues to report for the xbox version.

When upgrading a piece of armor or weapon with the stones and armor scraps the game will crash back to the xbox desktop home screen and completely close out. Happens more often when using multiple, one after another upgrade items.

Game starts stuttering and lagging after about an hour of play, most of the time it will start to stutter then slow down and crash back to the login screen. This happens fairly regularly.

Sometimes, not very often, when mobs are right on top or extremely close to your character they cannot be hit without moving away and approaching the mob again.
