How Does Life Leech Work?

I have the Vitality Void notable and the two leech nodes leading to it, plus Vaal Pact, but my life isn't moving when I hit things. I can fire into a pack of forty creatures and gain not a single point of life.
I'm using Hrimburn gloves and Ice Shot, so I do zero physical damage; is that why I'm not leeching? Do I need to do physical damage to leech? Highest Character: Level 92 86% Block/Static Strike Champion - Abyss League I get bored of the grind around 90, probably won't ever get a higher character. Last bumped on Sep 22, 2017, 4:06:52 PM
