Father GGG & Mather Tencent, how to complete all ears and no stone unturned?

I have reviewed list from poe wiki and found there are 126 in total, not 128. Bugged?
I am currently 124/128 in tencent and seems lost 1 vaal lettre and dark tome.
Since the warehouse district was deleted during 3.0, maybe GGG missed 2 environmental lores in it.
There should 126 environmental lores for achievement "no stone unturned", not 128!
