off hand wep just disappeared, gems and all

During the beginning of act 3 "Cruel" I died from a Essence fight, and when I came back, to finish the encounter, my off hand wand and the gems in it were, gone. I did nothing at all they just were not there! what happened? I can surly not play with this kind of random bug. I am not threatening it is just too much of a time waister if periodically I loose my main weapons and socketed gems. Is this a known issue, and can the web be returned in some way?

my initial guess was that you accidentally pressed x to activate the alternative weapon slot..

Yeah OP, 99,9% sure that you pressed X and made a weapon swap !!
