It looks like you have two graphics cards, so the issue may be that Path of Exile is detecting the wrong one. This is usually because you're using a laptop with both an integrated and dedicated graphics card, and the laptop is switching between these as it needs to.

If this is the case, the "Adapter" dropdown in your Options menu will only show the integrated card, even if the game is using the dedicated card at the time.

You can take a look at this thread for a couple of steps that explain how to specify which card Path of Exile uses.
