How does Life Gain on attack work?

+x Life Gained for each enemy hit by your attack, how does it work?

I tried using Freezing Pulse with it but gained no health. That means that I must do psychical damage to activate it?

If it says attack and it doesn't work with ANY attacks they should change it to physical damage.
Freezing Pulse is a spell, not an attack.

at the top of the gem if it says attack it will work, if not it wont

That makes no sense, by definition using a spell to ATTACK someone would imply implicitly that it is an attack. ANYTHING is an attack because you are ATTACKING an enemy.

I am looking at it from the randomly rolled stat on my weapon.

I agree with your logic, but the game doesn't. That's just the way it is. I think they could use better wording to make this a little bit more clear.

Ice Nova is "Cold, Spell, AoE"
Burning Arrow is "Fire, Attack, Bow"

I'm using Ice Nova to attack an enemy, yes, but I am doing it via a spell;
whereas with Burning Arrow, I am attacking the enemy with an attack (which uses the weapon's damage, unlike spells)

Nice one :) I forgot you can always tell what type of damage or support a skill is modified by via the wording at the top of the skill.

Man, that is one slippery slope of word definitions. I get it now, but still, wish they would clarify it better so misunderstandings would occur.
Every time you hit an enemy with an Attack, you gain the listed amount of Life. Damage is irrelevant.

GGG has been quite consistent with their use of 'Attacks' versus 'Spells'. Admittedly it's a bit unintuitive that it doesn't correspond to the informal notion of 'attacking' (i.e. trying to damage/kill) a monster, but it's difficult to see how to phrase it better.
