Horrible latency issues

Your latency and packet loss issues are starting very early in your trace, as early as your computer. Do you run any other programs while playing Path of Exile? Do you share your network with any other users? I'd start by checking your Firewall and Antivirus programs are not filtering Path of Exile and using a wired connection to your modem/router.
