Cast when damage taken and Curse on hit - whats up?

I have problem with this 2 skills.

1) Curse on Hit + Flammability, this curse Flammability never was used with this support game. Why?

2) The same, immortall call or molten shell never was used with cast when damage is taken. Why?

1. You need to link Curse on Hit to something that hits.
Your Spells, your Attacks, whatever you're using.

2. Read the description of Cast when Damage Taken.
It's level is too low to support IC and EC.

2) But why it doesnt use my molten shell? I get this damage but it still not using.

Your immortal call and enduring cry are both level 60. Your CoDT is level 59. Use a scouring and a skill gem to lower it. Or level your CoDT one more.
