Frenzy charge bubbles not appearing
I was running around with Blood Rage, and for the entire time this specific Blood Rage buff was recharged, the Frenzy charges were not showing up around my exile. The Endurance charges still showed up as you can see, and once the buff finally wore off, the next Frenzy charges appeared like normal.

No, I've been trying to reproduce it, this might have to do with the explodey purple guys in the Western Forest and the timing of Blood Rage.
The buff icons remained functional, and Flicker Strike would use charges as normal. The enemies had their own Frenzy charges while mine were not showing. |

I was finally able to reproduce it today, so one of these factors may lead you to the root of this tiny problem.
-Internet was saturated with torrent traffic. -My spiked shield instantly kills low level skeletons here with reflect damage. -I was literally in the casting animation for Blood Rage when this happened again. I think the exploding purple dudes and these self-killing skeletons were somehow causing the blood rage charges right as it was cast, at least that's how it appeared in each situation. Screenshot:

" I can reproduce this bug, but not consistently right now: - Activate Blood Rage - Fight mobs and build some some charges - Go to a large mob - Hold down the Flicker Strike button Things happen quite quickly, but in some cases your charges will disappear while holding down Flicker Strike, even though your charges are still usable and still show up in the top left of your screen. Also another bug that happens very often is at 7:20ish.. The character turns white while in blood rage. Maybe the alpha channel of the BR effect? While it's probably a bug I kind of like it.. Try to ignore the lag.. that was mostly on my side in this particular video. |

I've been having a different problem with frenzy since several patches ago. I'm using a wand build and frenzy damage is delayed by roughly a second, killing its efficiency as I wind up wasting mana on enemies that should have died three frenzy attacks prior. Even more frustrating is when the enemies manage to get an attack off despite effectively being walking dead. The funny thing is, I used frenzy for bows and a sword and board build and had no issues other than some difficulty balancing damage and mana costs.

Okay, the video helped a lot, we'll be able to investigate further now. It is probable that it will only happen if you've got a spike of latency occurring when you use up all of your charges.
