Act 2 Quest - Through Sacred Ground

I recently joined a lower level friends game who was in the middle of The Sacred Hand quest in Act 2 normal difficulty. My lvl 43 Shadow , who must have already done this quest before - that is in Ruthless difficulty, was able to pick up the hand and turn the quest in for the two skill point refunds.
Im pretty sure i had already done this quest in normal difficulty.. (dont you *have to* to progress through the entire act??) Anyway i currently have 10 skill point refunds and i dont even know what i had before that or what i may have used already. Perhaps i just saw the text for the turnin / quest reward but it didnt actually give me anything. You wouldn't have done the quest - It's not required, you probably just didn't notice! It's only possible for a quest to give out it's reward once!
