Chaos Recipe not working for me

I cannot figure out what is wrong? It worked before.
Full set all ilevel60+

I have double triple and quadruple checked all the items for i level.



Any ideas?

I'm fairly sure the fact that these two have the same name is what's causing your problem. Do you get offered one chance orb when you try to get the chaos?

I did not know that the names that spawned on chests could also spawn on helms. I always thought there was a certain set of names for each gear slot. Huh!
