Breach encounter spawns no monsters

Did a breach encounter in campaign, even thoug i run around making sure all paths are clear and doors opened, when i start the encounter, zero monsters spawn, and other times they're stuck where they spawn, never moving towards the centre, i have to run to them to get them to move


This has happened multiple times, resulting in little no no rewards
