Omen of amelioration not working

I've had the same Omen of amelioration in my inventory since a week ago. I've died 3 times in that time. The omen is still there. It's still on, so no, that's not the problem. After each death, I turned it off and on to try and "unbug" it. Died again 5 minutes ago, its still there, and I'm still losing 10% exp.

-Every death I was shapeshifted wolf.
-Every death on maps.
-This last time, I also had Omen of Resurgence on in my inv, it didn't work either.
-They are always on, but they don't trigger and aren't being consumed. It's like they are not there.

-I'm tired of this.
