Vaal Beacon did not activate after surrounding monsters are killed.

I was playing a tier 9 Penitentiary, and after killing the monsters surrounding the Vaal Beacon, it did not activate. It did not bring up a chest, nor create more monsters. I moved on and killed the boss, and after there were 0 monsters left on the map, the beacon still did not react.
