Quest soft-lock after defeating Kavakai – Hooded One ritual does not trigger

Description:
After defeating Kavakai, the Hooded One appears but the ritual to cure Kavakai and Makoru does not trigger. NPCs remain frozen with no dialogue or interaction.

Result:
Quest step “Witness the Hooded One cure Kavakai and Makoru” does not complete and the next main quest does not unlock. Character is permanently blocked.

Steps tried:

-Full logout
-Game restart
-Re-entering area

Expected result:
Cutscene/dialogue should play and quest should advance.
