Offhand item disappeared after server crash during Vaal Temple

On 13.12.2025 at approximately 1:30 AM Polish time CET, while clearing Vaal Temple as part of a league mechanic, the game servers crashed or disconnected.

At the moment of the crash I was playing a Druid and was in Bear Form. In my offhand I had the talisman Hysseg's Claw Familial Talisman equipped.

The servers were unavailable for a longer period of time, so I logged off and went to sleep.

When I logged back in on the same day at 11:47 AM Polish time, I noticed that my character was still inside Vaal Temple but was no longer in Bear Form and was instead in human form. At first my skills were not working, which confused me. Shortly after that I noticed that my offhand item was missing.

The talisman Hysseg's Claw Familial Talisman completely disappeared. It is not in my inventory, stash, or anywhere else. It appears to have vanished due to the server crash or a character state desynchronization.
