POE2:Treasures of Utzaal, Act 3 Molten Vault, Quest item not dropping

Defeated Mektul, the Forgemaster twice and the Quest item Hammer of Kamasa Does not drop, Turned off loot filter and all that to make sure that wasn't the issue. Recleared Vault a 2nd time and still doesn't drop

Tried resetting the instance 3 times with no drop. |

This has happened to me as well done it 3 times now cleared all enemies, Hammer of Kamasa is nowhere to been seen. Anyone know how to fix this ?

restarted the game and cleared the vault twice. portaled out and came back to see if anything changed. disabled loot filter with alt and checked again. no hammer dropped. |

Same..it worked fine on other characters, but on my chaos dot witch, I have tried 3 times and the forgemaster refuses to drop the quest item
