Blasphemy keeps turning off

Whenever I logg or use a portal, Blasphemy stops working. I have to go into my skill settings and turn it off then on again for it to work. Very annoying.

Socketed with Temporal Chains, Ritualistic Curse, and Encumberence, if that helps determine the issue. Playing a Lich.
