Omen of amelioration not consumed on death

Had the omen not get consumed a few times, I'm not sure if it's prevented the exp loss because of that. I noticed most recently I died to some form of dot damage. Since it's not the first time I'm beginning to think Poison/ignite/bleed causing death may not be consuming this?

Only one Omen can be used per area. If another Omen of any type triggered in your zone, any other Omen will not work.
