Bug with Omen of Amelioration

They randomly decide not to work. You die and lose exp and they are not consumed. Not sure what the specifics are of them. The last two times was during a ritual fight, and another time in trial of chaos.

Also have been experiencing invisible explosions/blood pools in trial of chaos.

Only one Omen can be used per area. If another Omen of any type triggered in your zone, any other Omen will not work.

Yes. All the combat-area-related Omens say this, even the strongbox/shrine/rogue exile etc ones.
