Logbook wiped after defeating boss

I did a logbook that contained a boss. I went in a killed the boss before looting anything else.

After beating the boss I warped to my hideout to dump my inventory. I returned to the logbook and exited the boss area. I found that everything in the surface area had been wiped out. Even two other underground areas that I know my chain hit were reverted to a closed state even though the mini map still shows them.

All my chests and loot are gone. I can't even interact with the NPCs.
