Chimeral Inscribed Ultimatum - Will not get consumed by altar

Hi there - I've been stuck with the Chimeral Inscribed Ultimatum in my inventory for weeks now b/c it will not get consumed when running a Trial of Chaos. I can place it in the statue/altar and start a trial run, but 1) I will not get any soul cores from the run and 2) the quest version of the Inscribed Ultimatum (Chimeral Inscribed Ultimatum) is still in the statue after completing the run. Any tips as to how I can fix this?

Maybe related, but if I'm an instance owner and I try to run any non quest Inscribed Ultimatum for a Chaos run with a party, I get locked/stuck at the loading screen. The only way it will let me load the game back up is going AKF for 15+min and coming back where the instance closes...
