Bug where giving only the Green mushroom to the Cauldron Keeper (Act 3) completes the objective.

The message upon picking up the Green mushroom first as a drop makes the character say, "And that's three." even though the other 2 mushrooms missing never dropped and were never collected.
I was playing on Warrior/Titan when this happened. When game developers ignore the criticism that would improve their game, the game fails. Just because a game receives a great amount of praise vs. only a small amount of criticism does not mean to call it a day and make a foolish misplaced assumption that it is perfect.
