[BUG] - Iktab's Permanent Storm Orb (Phaaryl Monolith)

Within the Phaaryl Monolith endgame zone encounter; if Iktab the Deathlord (ACT II boss) happens to spawn and dies while his storm orb is still up - the orb itself will permanently remain within the boss arena (thus making this fight far more difficult due to such a small space).

EDIT: Also an additional bug with this zone; if the player leaves the area to disenchant stuff before opening all the runes, then they'll become permanently unlootable.
