Omen of Amelioration Fails to Activate Upon Death in *Path of Exile 2*

Dear *Path of Exile 2* Support Team,

I am writing to report a critical issue I’ve encountered with the Omen of Amelioration item, which is severely impacting my gameplay experience.

Problem Description:
When my character dies while the Omen of Amelioration is actively equipped in my inventory, the item *does not trigger* as intended. Despite being marked as "active," it remains in my inventory after death and does not consume itself to prevent experience loss. As a result, all accumulated experience is permanently lost, even though the Omen itself is not consumed. This issue occurs with a *very high frequency* (nearly every death), making it functionally impossible to progress beyond level 91.

Expected Behavior:
As per the item’s description, the Omen of Amelioration should activate upon death, consuming itself to protect a portion of the player’s experience. However, this is not happening in my case, rendering the item ineffective and causing repeated, irreversible XP loss.

Impact on Gameplay:
This bug has made it *impossible for me to reach level 92*, as any death results in total XP loss despite the Omen being active. The issue persists across multiple attempts and sessions, suggesting it is not an isolated incident.

Request:
Please investigate this problem to determine:
1. Why the Omen of Amelioration fails to trigger upon death.
2. Why the item remains in the inventory instead of being consumed.
3. Whether this is a known issue or related to specific gameplay conditions (e.g., item interactions, server delays, or character-specific bugs).

Are you using other Omens in an area? Only one Omen can trigger in each combat area.

It appears that even though I confirmed it was active, after dying, it is not consumed and remains active in my inventory.

Same here I've stopped buying them, they fail to activate.

I specialy checked it. Then quntity of mobs is low and you die, or its not a post death monster skill- omen is use. But magma marrier of rarity mobs is kill you and omen is not have result.

I lost my advance, time, and currency because of the omen was clicked but not self activate.
