How exactly can one obtain the item called "Sacrificial Heart"?

Oh my god, finally it's done. I managed to obtain my quest item, Sacrificial Heart, by disabling the Herald of Blood aura and minimizing the damage from bleeding. Oh my god, this bleeding damage or the Herald of Blood aura might be bugs. The Herald of Blood might prevent my quest item from dropping by making the monster corpses disappear, and the bleeding damage might be caused by the fact that bleeding doesn't count as my character's kill, so it doesn't allow me to obtain the quest item. Please, gg, can you check why the bleeding damage and Herald of Blood aura cause the Sacrificial Heart quest item not to drop? (I started from level 62 and reached level 64.)
