Freythorn final Ritual Altar not unlocked

Cruel mode.

Entered, cleared the zone, including the initial 3 altars, and the final altar remained locked. I did note that when I approached the first altar the NPC summon bubble wasn't there ... it appeared after I invoked the first altar, so perhaps that sheds some light on the bug.
