PoE2 Altars and Unaccesible Map Areas

3 Altars in a map, none of them can be activated

There was also a rare monster in an area of the map that isn't connected to the rest, as well as a different area which was simply walled off.

I think I figured out why altars get bugged, it's the sandworms that hide underground until you get close to them, either they run away somewhere and you don't notice or they get spawned in a out of bounds area when the altar is right next to a wall, or killing them too far maybe don't count towards altar.
