Character Progress Reset, Map Failed, Breach Lost

I was clearing Sulphuric Caverns in a Breach, and after clearing a group of Breach mobs, the game stopped. I could hover over items and see the details, I could also hit ESC and go through menus, but I couldn't move. I think there were mobs off to the edge of my screen and they were not moving, not even running in place. I had just hit Level 86, I hadn't assigned my point yet. I hit ESC and selected Resume, but that did not fix the issue. I then hit ESC and selected Exit to Character Selection. My Titan is now back to level 85, at %99, and the map node now says, "Attempted" and no longer has the Breach icon.

I did have the performance graphs up while this was going on, and the green Network line was all the way at the top, and was a series of dashes, instead of a solid line.
