Game freezes during Breach encounters

Has happened several times and only during active Breach encounters. The game completely freezes up and you are forced to restart the game. All the items in your inventory are lost, as is the waystone and the map node is considered "failed" if you haven't completed it before the crash happens.
Might be unrelated but each time it happened I was about to attack a rare monster. Playing as Ice Strike monk, utilizing freeze and shock ailments.

Same with my Ice Strike Monk. 4-5 times today.. Bump!
