act 3 ruthless Jiquani's machinarium stone altar bug with controller

I have the small soul core.
I'm standing right in front of the altat but pressing A does nothing.
I have no skill binded to the a button.
I already talked to Alva.
The quest says to place the small core into the altar.
It's not working.
I even tried teleporting out and retrying but it's not working.

It looks like there are enemies right on the other side of the door most likely preventing me from using the A button to interact with the altar.


Resetting the instance and placing the core into the altar before summoning Alva worked.
