Constant server crashes and rollbacks due to Breaches

Every 1/3 Breaches ruins a map for me. Exactly 15 seconds after opening it the game freezes, latency spikes, and I'm kicked back to the main menu. When logging back in, I'm completely rolled back. Nothing I looted is in my inventory, all my exp gained in the map to that point was removed, but the map is gone and every buff on the atlas is removed. Like the game snapshotted the point where the map was consumed but before any portals appeared, and just assumes I failed.

I'm not dying, my death counter never goes up when this happens. Breach is the only mechanic that causes this.

It's happened on a wide variety of maps, there's no consistency for what's causing this server crash.
