Beira of the Rotten Pack Reward Head of the Winter Wolf Not Dropping

I am at the end of the game and my friends explained I should be at -40% resistances if I got all the campaign quest rewards with no gear on.

Sure enough my map says I am missing the second Beira of the Rotten Pack.

Went and killed her and no drop happened. I do not have the quest reward in my bank either or inventory.

Need the additional 10% cold resist to literally play the game on equal footing.
