MOLTEN VAULT QUEST BUG

MOLTEN VAULT QUEST BUG!!!!

If you kill the end boss and die from one of his exploding spikes at the same time, and before you collect the hammer for the quest, you will respawn up top at the checkpoint, and you are forced to run molten vault all over again because the quest hammer spawns in the molten river pit with no way to get down there.

No you cannot swap your input to try and grab it.
No you cannot destroy the bosses body and reload checkpoint to restart fight.
No you cannot re-activate the crane to lower you down.
