Crash in both Trials (POE 2)

I completed the third ascension trial on my Witch in PoE 2, running the Trial of Sehkemas. Upon completion my game crashed, and I didn't get my ascendancy points. On reload, both the trial map and trial were lost, with no way to re-enter.

I then moved to the Ultimatum trial, and one of the fire mobs jumped on my character and started the frontal-cone flame breath, which crashed my game and also bricked that trial. Upon reload I cannot get back into that trial and the trial map is lost.

My game crashed on exactly same spell in ultimatum. More than once. Lost several trials, frustrating.

Attempted another Trial Ultimatum. This time the Bird Boss (first boss) did a down slam beak attack while both the blood ball and blood tether abilities were queueing at the same time. Game crashed and bricked my trial and could not get the Ultimatum key back.

I am playing a Summoner Infernalist.
