Low FPS even at 240p resolution

Hello, i get low fps in combat around 40 FPS. even when i downscale all the way down to 240 resolution(which is weird there should be some change). no matter what settings i pick my FPS doesn't seem to improve or change. 90 FPS out of combat 40 FPS in combat even at 240p.
My friend with a extremely similar specced out PC has around 120 fps in combat.
Specs : CPU : intel 9900k
GPU: 2080TI
RAM: 16GB
Storage: Samsung Sata SSD
Nvidia Drivers up to date.

I know theres alot of fireballs this is an extreme example i chose but i get 40FPS even when not much is on screen.

any suggestions would help.
Things i have already tried :
1- changing all the settings in game including dynamic culling
2- changing windows graphics settings to give high power to PoE.exe
3- Nvidia settings changing OpenGL to high performance.
