Cast on Minion Death & SRS get Deactivated after triggering in some scenarios [video linked]

It looks like in some scenarios SRS and Cast on Minion death are getting deactivated after triggering occasionally. I can recreate this regularly, though it sometimes takes some time (5 min tops) to do so.

to recreate:
1. Use SRS with infernal legion and minion instability

2. Use Cast on Minion death with something. I have tried both Comet and Firestorm with Spell Echo and Spell Cascade.

3. Cast Fireball or Ember Fusillade (i am using wildshards on mine) to trigger cast on minion death

On further play it looks like it could be BloodMage specific. I am having similar issues with minions (specifically skeletal clerics) and it seems blood remnants might be messing up the spirit reservation.
