Buff bar lists multiple Infernal Hounds

The buff bar lists multiple Infernal Hounds, but only one is following me.

Character: Hellfyer

I guess it might have to do with some of the following things:
- Last Gasp on Hound
- Minion Pact on another skill
- 85% increased duration on tree
- Minions Revive 15% faster on tree
- +1 to Minion skills on (automatic) swap

The Infernal Hound also seems to stagger whenever I weapon swap, I'm guessing due to +1 to Minion skills on the weapon swap.
