POE 2 - Cannot Complete Lieutenant/ Nameless Seer Maps

Upon clearing a lieutenant map and the boss, the map does not complete and locks progression from adjacent nodes. The same thing occurs after selecting a unique from the nameless seer. After starting a new node, the map will show "Map Failed"
