Act 2 cruel Trial coin gone

I went into the sanctum for my third ascendancy, but sadly died out during the twin boss of floor 2. Now my coin is gone and I can't get a new one. Tried killing Balbala again but did not drop a new coin and talking to her won't do anything either.

so u cant get 3 ascedant right? i have same problem
