Dying at the same time as Count Ogham Act 1 boss makes loot disappear

I died within 1 second of the Act 1 boss Count Ogham dying. I listened to his monologue and watched The Hooded One finish him off but when I respawned and reentered the arena his corpse is gone and the loot as well, including the uncut skill gem quest reward. It did count the boss kill for the quest completion and I was able to travel to Act 2.

Not just that boss. ALL bosses are having this game-breaking bug. Mandatory Quest Items are also disappearing and I cannot progress with the story.
