Esh Breachstone spawned no monsters

Bought a normal Esh Breachstone from the Currency exchange. Used it pretty much right away.

After entering the Breach, I went over to the hand but it did not spawn any monsters. There was only 1 tormented spirit in the map.

Not sure what happened, timer just started with 40 seconds on the clock, the hand spawned no wave for the enemies.

Wasn't doing anything before this. Just fired up the game.
