Mana regeneration stopping completely

After grabbing 4 eldritch altars, my mana stops regenerating. Running a Hiero Ice Nova of Frostbolts build.

Are you referring to after you take the altar shown in the screenshot, that has "Player gains: reduced mana recovery rate per Endurance Charge"?

With 4 minimum and maximum Endurance Charges, that modifier would give you -72% mana recovery rate. Note that it's recovery rate, not regeneration rate, so modifiers increasing mana regeneration rate would offset the penalty. There aren't many sources of mana recovery rate in the game, so that altar downside would cut your mana regeneration drastically.
I don't see the buff in the screen shot. Not sure why I'm having issues with the screenshot displaying from imgur on my end but I typed out what I had when the mana stopped regen'ing.

Player gains:
-48% additional Physical Damage Reduction
All Damage taken from Hits can Sap you
32% chance to be Sapped when Hit
Non-Damaging Ailments you inflect are reflected back at you
Projectiles are fired in random directions
=
16% increased Quality of Items found in this Area
39% increased Rarity of Items found in this Area
Maps dropped by slain Enemies have 33% chance to be Duplicated



Eldritch Minions gain:
42% increased Attack Speed
35% increased Cast Speed
Skills fire 3 additional Projectiles
36% increased Movement Speed
Inflict 1 Grasping Vine on Hit
Drops Chilled Ground on Death, lasting 3 seconds

2.8% chance to drop an additional Chromatic Orb
2.5% chance to drop an additional Jeweller's Orb
Out of curiosity, did you pick any altar mod from the shown ones? You're saying the regen stopped before you picked the altar, but I wonder if there is a bug where those mods apply before you pick the altar option.

So if you picked the top option that did not reduce mana regen, I wonder if your mana regeneration started again. Still a bug, but could be the source.
The mana regeneration stopped after the grabbing the fourth altar. Can't remember if one of those altars had the "With 4 minimum and maximum Endurance Charges" mana reduction mod as the other option. I ran the rest of the map because I got mana from kills (Mind Drinker anoint) but had to turn off my automation/arcane cloak because it drained my mana down to nothing.
