Retaliation Skill Character Icons not displaying properly

My character uses three retaliation skills: Crushing Fist, Eviscerate, and Glacial Shield Swipe.

The latter two, Eviscerate and GSS, seem to share an icon above the player's head. When all three are available, like in the screenshot above, only two icons appear. If only Eviscerate and GSS are available, only one icon is displayed, seen here:



Then, if all three are available and Crushing Fist and one of the other two are both depleted, there can be no icon but still have a useable retaliation skill, as seen below (one for GSS, one for Eviscerate).
