Beginner Questions About Boss Carry

Is there a "getting carried for dummies" guide somewhere? (I tried to find one.) If so, point me there. If not, some questions:

1. Do I have to be in the zone when the boss is killed to get my voidstone? (Maven)

2. If yes, do I have to be alive?

3. If I die, should I run back in right away or wait for them to signal?

4. What's the customary way to handle loot? For example, short allocation and grab anything I want?

4: discuss it beforehand. the most common agreements are: cheap/free carries but they get the loot, or the carrier takes X payment and all the loot is yours

Usually the person who carries, does damage until the boss has low health and then writes in the team chat that you can enter the zone. If you don't move in the boss zone, you are invulnerable for a period of time (around 40 seconds) upon entering. There is a small icon upper left, which shows the period.
