Puppet Mistress optional quest bug

Restarted game after this happened the first time and then it occurred again. Mistress quest is incompletable because she does not spawn/is not alive on entrance to the Spawning Grounds.

She's never active/alive on entrace to The Spawning Grounds.

There are three spawners that you have to kill on the lower level of the area--one on the left, one on the right, and one in the bottom middle--before she becomes active.
