Added damage from arcane cloak

Is the added damage from arcane cloak applicable to all sources (of hits)? So spell, attack and special (but not DoT)?

It works with both attacks and spells, yes. It even works on glove enchants, not that that's relevant to anyone but standard players now.

Yes, totems use your offensive stats. They have their own defensive stats and would not benefit from (nor drain) your shield.
