Omen of Amoreliation does not trigger if you have already died in that map

I have been running maps trying to level to 100 and I buy Omens 5 or 6 at a time so I always have one if I die.
On more than one occasion I have died in a map (in both cases it was a t16 Tropical Island) and my first Omen of Amoreliation pops on death. I then grab another Omen from my stash and head back to the mob that killed me. The mob kills me again and the fresh Omen I put in my inventory does not pop and I lose the full 10% experience. Only one Omen can be used per player per area. This is said on the Omen.
