disconnected - this operation requires you to be logged in

The last week I have been getting this issue which I have never seen before, and it seems to be getting ever more frequent, to the point that it seems 50/50 if I can successfully join an instance.

When I then log back into the game it puts me randomly into an act town, so getting logged in, then to my hideout and then actually into my map is becomming a complete nightmare, and then I almost never manage to run more then 2 maps before I have to go through all this hassle again.

Trade also then becomes a real head ache as I might have to log into the game 3-4 times just to get from my hideout to a seller.
