Omen of Amelioration didn't trigger

Had one trigger a few mins before no problems... But second one didn't trigger on death. Above is the map I died on.


Went back to spot where I died and it might have been these guys, but can't be sure.



Swapped the "faulty" omen out from my inventory for another one incase that is the problem, but unsure :s

More in depth explanation of events:
Only had one Omen in my inventory on death for first death, then swapped in a new omen to the same spot of my inventory upon respawning.
Headed back to the map cleared a bit further and died again, but the new omen didn't trigger and was still in my inventory on respawn.

Swapped omen out for another omen incase it's the specific item that's bugged and then submitted bug report in game.
Pushed back to spot took second screen shot of enemies and finished map.

Now going to restart game.


Also changed loot filter a bit before this during this login. Unsure if this could be tied to it, but just in case.
